Does the person want to come to FiND?
Visits/ taster session to be arranged.
What does the person want and can we meet your needs?
ASC assessment/Brokerage information is to be provided at this stage. If FiND are unable to meet your needs, where possible we will try and signpost to services/organisations that may be able too.
FiND’s consent forms, privacy notice and T&C’s to be issued. FiND’s initial assessment and ‘My Plan’ to be completed with the person/family/carers, professionals and important people. Without these documents in place, a service cannot be provided.
Staff matching to be completed. Specific training to be completed.
Medication documents and plans to be in place.
Start date to be agreed.
(Please allow 8 weeks from funding confirmation to start date)
